The same sky, different rulebooks
A private pilot taking friends to lunch, an air ambulance crew, and a Boeing 787 crossing the Pacific are all flying in the same airspace under the same air traffic control system. What differs is the operating rule they are held to.
Part 91 is the baseline: general operating and flight rules that apply to every civil flight. It is the least prescriptive of the three, and a private pilot operating under it makes almost every decision themselves.
Part 135 governs commuter and on-demand operations — charter, air taxi, air ambulance, small scheduled services. It adds requirements on pilot experience, recurrent training, maintenance programmes, duty limits and weather minimums.
Part 121 governs scheduled airlines and is the most demanding regime in civil aviation. Its requirements touch nearly everything: how the aircraft is maintained, how the crew is trained and checked, how long they may work, what weather the flight may depart into, how much fuel it must carry, and who signs it out.
The pattern behind all three is simple. As the number of people carried grows and their ability to assess the risk they are taking falls, the regulatory burden rises. A passenger buying an airline ticket has no way to evaluate the operation, so the rules do it for them.

The ladder of certificates
A student pilot certificate allows training with an instructor and, once endorsed, solo flight within specific limits.
A private pilot certificate allows carrying passengers, but not for compensation or hire. In the United States it requires a minimum of 40 hours of flight time, though the national average to reach the standard is substantially higher — commonly cited around 60 to 70 hours.
An instrument rating is added to a certificate rather than replacing it, and permits flight under instrument flight rules in instrument conditions. It is the single largest step in capability a pilot makes, and it is where the material in chapter three becomes a licence rather than an idea.
A commercial pilot certificate permits flying for compensation. In the United States it requires 250 hours under Part 61, or 190 under an approved Part 141 school syllabus, along with a higher standard on the flight test.
An airline transport pilot certificate is required to serve as a pilot for a Part 121 airline. The baseline requirement is 1,500 hours of total time — with reduced minimums available through specific paths: 1,000 hours for graduates of certain approved four-year aviation degree programmes, 1,250 for certain two-year programmes, and 750 for qualifying military pilots.
Alongside these sit the instructor certificates, and they are the reason the system works. Most pilots build the hours between commercial and airline transport by instructing, which means the profession trains its next generation using the people who have most recently learned it.

Who is actually in charge of an airline flight
Under Part 121 the captain is still the pilot in command with final authority, exactly as in chapter one. But they no longer plan the flight alone.
A licensed aircraft dispatcher, sitting in an operations centre, plans the route, computes the fuel, checks the weather, considers the aircraft’s maintenance status and issues the flight release. The captain and the dispatcher share legal responsibility for that release. Both must agree that the flight can be conducted safely, and either can refuse it.
This is a genuinely unusual arrangement and it exists for a specific reason. A captain about to operate a flight is subject to a set of pressures — schedule, passengers waiting, the crew’s duty day, the cost of a cancellation — that a dispatcher is not. The dispatcher provides a second, independent judgement made by somebody who is not standing in the aeroplane.
The dispatcher also stays with the flight. If weather changes at the destination while the aircraft is airborne, the dispatcher is watching it and can amend the release, suggest an alternate or supply information the crew cannot obtain from the flight deck.

How much fuel is enough
Under Part 91 the minimums are modest. For VFR flight by day, enough fuel to reach the first point of intended landing plus 30 minutes at normal cruise. By night, 45 minutes. For IFR flight, enough to reach the destination, then the alternate if one is required, then 45 minutes beyond that.
Whether an alternate is required under IFR is decided by what is usually called the 1-2-3 rule: an alternate must be filed unless, for at least one hour before to one hour after the estimated time of arrival, the forecast shows a ceiling of at least 2,000 feet and visibility of at least 3 statute miles.
These are legal minimums and, as with everything else in this course, they are floors. A 45-minute reserve sounds comfortable until it is being spent holding for a runway that has an aircraft stuck on it.
Part 121 fuel planning is considerably more elaborate, adding contingency fuel for route inefficiency, holding fuel, and taxi fuel, all computed by the dispatcher against the specific route, forecast winds and aircraft. The difference in philosophy is worth noticing: Part 91 gives a pilot a number to beat, and Part 121 gives a dispatcher a calculation to perform.

Reserves are computed at normal cruise consumption. 14 CFR 91.151 and 91.167. Note that these are the legal minimum on landing, not on departure — arriving with less is a violation, not a close call.

The pressure that has no name on the checklist
Every commercial flight carries a pressure that a private flight does not: somebody is paying for the outcome. Passengers have connections, freight has deadlines, an air ambulance has a patient, and a charter customer has an opinion about delays.
This is not a character flaw in commercial pilots. It is a structural feature of doing the job, and the industry has responded by building structures that resist it rather than by asking people to be stronger.
Dispatch release is one, and the reason it works is that it moves half the decision to somebody standing somewhere else. Duty and rest limits are another: they are hard numbers precisely because a tired pilot is a poor judge of their own fatigue, and a rule that said "do not fly when too tired" would be no rule at all.
The sterile cockpit rule is a third, and it is the most specific. Below 10,000 feet other than in cruise, non-essential conversation and activity are prohibited on the flight deck — because the phases of flight where most accidents happen are also the phases where the temptation to chat is highest, and because the rule removes the social awkwardness of one pilot having to tell another to be quiet.

The last leg of a long day
You are the captain of a regional jet on the fourth leg of a four-leg day. Your duty day began at 05:40 and it is now 18:20. You are legal on duty and rest — with about 90 minutes of duty remaining.
The flight is 55 minutes. Sixty-eight passengers are boarded, several with connections at the destination that are already tight because of a 40-minute delay earlier in the day.
The destination forecast has deteriorated since the morning: a line of thunderstorms is expected to reach the field around your arrival time. Your dispatcher has filed an alternate 90 miles away with good weather and has planned fuel accordingly.
Decisions stand. You will not be able to change one once it is made — fly the mission again if you want to try a different route.

The dispatcher calls. The destination is now forecasting the storm line arriving 20 minutes earlier than planned — likely just before your arrival rather than just after.
They offer two options: depart now with the current fuel load and accept the possibility of diverting, or take on an additional 25 minutes of holding fuel, which requires a fuel truck and about 20 minutes on the ground.
